Survey Planner has been created so that, as an admin user, you may create and edit Internet based surveys and questionnaires, then collect and analyse respondent's data and in due course, integrate elements of this data into your existing website and Internet strategy.

Understanding Fields & Creating Questions
The variety of question types that a user may be asked is limited only by your imagination. The type of answer you require from the user and the format that response comes in is determined by how the response requested.
SurveyPlanner allows you, the admin user, to set this format when a question is created. After selecting the ADD QUESTION button, you will be shown the following select list.

Each of these formats is explained below:
- Text Label - This is the default option, selected when you choose to make a new question within a survey. Unlike the other question types however, the Text Label does not request a response from the survey user. The Text Label can be used for introductions, descriptions, explanations, instructions and the like. To create a Text Label, select this option from the list and click the ADD button.

Insert the required text into the Article Editor area and select the save button to create the Text Label. The finished product will appear as a paragraph in the survey (dependent on the format you have assigned to the text). As this is not a true question - requiring a response from the survey user - when placed onto a page in a survey Text Labels are not numbered.

- A Check Box may be created by choosing Check Box from the select list when creating a question. When creating a Check Box question, you will be presented with a screen similar to the one shown below.

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page (as shown in the image above) then this question will be displayed as question number 1. Please note however that if such a question is preceded by a Text Label (which does not have a number assigned to it) the check box question will still appear as number 1 in the survey.
- Each set of Check Box answers requires some sort of question. Enter the text for this question in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Multiple answers may be added to a Check Box question. One answer will automatically be included in all newly created Check Box questions. If you wish to include additional options for survey users to select, click on the ADD button. Consequent options will appear below the first.

You may customise the text that appears next to each Check Box by altering the text shown in the Options column.
One or more of these Options may be set as checked. By default, none have been ticked. Click on the Check Box at the left of an option to have that option ticked by default.
To remove a Check Box option, click the DELETE button next to that option.
- When Other text is included in a Check Box question, the survey user may write in their own answer to the question (instead of choosing a predefined Check Box option).
Define whether or not the survey user should be given this free text option by selecting YES or NO from the Include Other Field select list.
As the admin user, you may also define what text appears next to this free text box. By default, this will be the word Other, however this may be customised and formatted as required.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, the Check Box question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- Minimum Number of Checks - When the check box field is marked as being Mandatory, a user must select no less than ONE of the check box options. As the admin user, you may define the minimum number of checks your survey users may make. The number of checks available in this select list will vary, dependent on how many check box options you have created in each question.
- Maximum Checks Allowed - If you have outlined in the check box question that you require only ONE answer, you should select the number One as the maximum number of checks allowed. Alternatively, the question being asked may allow for multiple answers to be given, if this is the case, select the number of answers required. If all options may be selected by the survey user, you should choose the highest number from this select list.
When there is at least one answer available in the check box question, you must select one or above for this option.
- Orientation - This option determines the appearance of check box options - as displayed to the survey user - when more than one check box option has been added. The images below show how the check box question appears on the page in Edit Mode.


- Estimation - This type of question allows a survey user to assign values to a set of predefined options. This question type may be used for estimations, valuations, ratings and the like.
To create an Estimation question, select Estimation from the select list when creating a question. You will be presented with a screen similar to the one shown below:

The next image shows how an Estimation question (with default settings) appears in edit mode. Unique to the Estimation question type is the survey user's ability to assign the numerical values next to one or all options.

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by a Text Label (which does not have a number assigned to it) the Estimation question will still appear as number 1 in the survey.
- Each Estimation object requires some sort of question. Enter the text for this question in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Answers to an Estimation question should then be defined.
- Text that appears next to a variable numerical value
- The numerical value - this field may be left blank if the survey user is intended to assign independent values.
- When Other text is included in an Estimation question, the survey user may write in their own answer to the question (instead of choosing from those that have been predefined) and then assign their customised answer a numerical value.
Define whether or not the survey user should be given this free text option by selecting YES or NO from the Include Other Field select list.
As the admin user, you may also define what text appears next to this free text box. By default, this will be the word Other, however this may be altered and formatted as required.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, the Estimation question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- TOTAL - the numerical value nominated here determines the total that all values a survey user enters may add up to.
- PERCENT - When set to TRUE, a percentage sign will be shown next to the numerical fields the survey user fills in. Selecting FALSE will remove the % symbol.
- TOTAL REQUIRED - When set to TRUE, the text Your total is currently: ___ out of ___ will be displayed under the question. For a survey user to submit or move onto the next page in the survey, the total value that they have entered into the numerical fields must add up to the value nominated by the admin user in the TOTAL field defined above. When set to FALSE, a survey user may move onto the next question without reaching this total value.
- Numeric - This question type asks the survey user for a single answer to be inserted into a text box.
Answers given by a survey user in this type of question must be a number (without spaces). If you wish to create a question like this, but accepting alphanumeric values (text and/or numbers) from the survey user, please refer to Text questions below.
To create a Numeric question, select Numeric from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by a Text Label (which does not have a number assigned to it) the Numeric question will still appear as number 1 in the survey.
- Each Numeric object requires some sort of question. Enter the text for this question in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- One answer field is available per Numeric question. As the admin user, you may enter a default value or leave this field blank. By default, when creating a Numeric question the value given in this field will be zero.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, the Numeric question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- MINIMUM REQUIRED VALUE and MAXIMUM REQUIRED VALUE - A survey user must answer questions of this type with numbers only. Here, the admin user may define how large/small the value entered by the survey user may be. In the example above, the value entered by a survey user must be above zero, and less than one thousand.
- ALLOW DECIMAL PLACES - When set to FALSE, only whole numbers will be accepted as input by the survey user. If the survey user enters a number with decimal places when the question has been set in this manner, they will not be able to submit or proceed to the next page in the survey. If decimal numbers are to be accepted in an answer to this question, select TRUE for this option.
- LIMIT DECIMAL PLACES - When decimal places may be entered by the survey user, you may wish to limit how many are used. To limit the usage to one decimal place, enter the number 1 into this field.
The image below shows how the Numeric question appears in edit mode when default values have been used to create it.

- Radio Button - This question type is quite similar to the Check Box question type described above, the main difference being that a survey user may only select ONE answer from the values available (including other). To create a Radio Button question, select Estimation from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by a Text Label (which does not have a number assigned to it) the radio button question will still appear as number 1 in the survey.
- Each set of Radio Button answers requires some sort of question. Enter the text for this question in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Multiple answers may be added to a Radio Button question. One answer will automatically be included by default in all newly created questions. If you wish to include additional options for survey users to choose from, click on the ADD button. Consequent options will appear below the first.
You may customise the text that appears next to each Radio Button by altering the text shown in the Options column.
To remove a Radio Button option, click the DELETE button next to that option.
By default, none of the answer options will be checked. Being a Radio Button, only one may be checked - as is the nature of this field type. If you wish for one of the defined options to be checked by default, tick the check box next to that option. If multiple options are checked by the admin user in edit mode, only the last of these will be checked when the question is saved.
- When Other text is included in a Radio Button question, the survey user may write in their own answer to the question (instead of choosing a predefined Radio Button option).
Define whether or not the survey user should be given this free text option by selecting YES or NO from the Include Other Field select list.
As the admin user, you may also define what text appears next to this free text box. By default, this will be the word Other, however this may be customised and formatted as required.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, the Radio Button question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- Orientation - In the same manner as portrayed in Check Box questions above, this option determines the appearance of Radio Button options - as displayed to the survey user - when more than one check box option has been added. Answers may be shown one on top of the other, or side by side.
The image below shows how a radio button question appears on the page when created using the default values.

Please note: once a radio button has been selected, it may only be deselected by clicking on another radio button option. Hence, if only one option is available to the survey user in a radio button question, they will not be able to de-select that choice.
- Scale - This question type allows the survey user to rate their answer/s on a scale. Multiple questions may be asked within a single Scale object. To create a Scale question, select Scale from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by a Text Label (which does not have a number assigned to it) the Scale question will still appear as number 1 in the survey.
- Each Scale object requires some sort of question or instructions. Enter the text for this in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Below the main question or instructions, each scale object may have one or more sub questions. Each sub question will be displayed along with a horizontal array of radio buttons. For each additional question that is asked within a single Scale object, a new line will be added to the display, along with an accompanying array of scale radio buttons, as shown below:

Multiple sub questions may be added to a single Scale object. One will automatically be included by default in all newly created questions. To create additional questions within a single Scale object, when in edit mode click on the ADD button below the existing questions. An additional question text field will show, along with the accompanying select list where any default scale option may be defined.

You may customise the text that appears next to each answer scale by altering the text shown each respective Article Editor.
To remove a Scale sub question, click the DELETE button above to that option.
By default, none of the radio button options will be selected. Being a Radio Button, only one may be checked - as is the nature of this field type. If you wish for one of the radio button options to be checked by default, in the SELECTED drop down list above that option choose the appropriate number.
- When Other text is included in a Scale question, the survey user may write in their own answer to the question (instead of choosing a predefined Radio Button option).
Define whether or not the survey user should be given this free text option by selecting YES or NO from the Include Other Field select list.
As the admin user, you may also define what text appears next to this free text box. By default, this will be the word Other, however this may be customised and formatted as required.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, answering a Scale question / sub question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- LEFT, MIDDLE and RIGHT IMAGE TEXT - When shown to a survey user, at the top of each scale question you will see a double headed arrow image. Above this image is text that may be used to describe how the numbers in a Scale are to be interpreted. Default values are included in each Scale question when it is created as Most Important, Average and Least Important. These values may be customised as required by altering the text shown in each of the respective Properties.
- RANGE - The value selected for this property determines how many radio buttons will be provided for a survey user for each sub question in the Scale object. Three options are available: 1-5 | 1-7 | 1-10 .
- SHOW DON'T KNOW, DON'T CARE and NOT APPLICABLE - Each of these fields may be shown or hidden as required. By default, all three are not shown. When set to YES in the respective select list, an additional radio button is shown next to each sub question's existing array of radio buttons. The image below shows one example of how this would appear in edit mode.

- ALIGNMENT - Text for each sub question may be aligned to the left or right. The image below shows how a Scale question appears on the page, in edit mode, when created using the default values. Alignment is set to LEFT by default. When set to RIGHT, sub question text will appear directly next to the array of radio buttons provided on that line.

Please note that Other text will not be formatted by this option; if you wish to edit the alignment of Other text you should adjust this directly in the Article Editor provided.
- Select List - This question type will create a simple drop down list, populated with defined values. A survey user may select one or more (dependent on admin defined settings) from this list when answering the question. There are two basic appearances that may be achieved when creating a select list, Single and Multiple line. For information on how to change your select list from a single to multiple line appearance and an example of each, please see NUMBER OF OPTIONS SHOWN below.
To create a Select List question, choose Select List from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by only Text Labels (which do not have a question number assigned) the Select List question will still appear as number 1 in the survey.
- Each Select List object requires a question. Enter the text for this in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Below the question, each Select List may have one or more answers. Each answer will be displayed in edit mode in two parts: Displayed Text and Saved Value. Displayed text is what appears in the select list - what is shown on screen. Saved Value may be the same as the Displayed Text for an answer, or it may be a shorter version of this information. Both of these fields must be filled in for each answer that is created.
As an example, a select list question might be "Would you like to receive a newsletter?". You may wish the answers in a select list to be as follows:
Yes, I would like to receive a newsletter
No, Please do not sent me a newsletter
While the Saved Values do not need to be this complex, and can simply be YES and NO respectively. To add an option to the select list answers, click the ADD button above the current select list answers.
To remove an option from the select list, click the DELETE button next to that option.

- By default, answering a Select List question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
For a select list to work correctly as a mandatory question, the first answer available to a survey user should always be created in the same manner as the default answer as provided when this type of question is initially created. This means, the first answer option should instruct a survey user to choose one of the other valid answers and the Saved Value for that option should be blank. All other answers and their respective Saved Values should be filled in as usual.
- ALLOW MULTIPLE SELECTIONS - By default, this is set to FALSE, meaning that a survey user may only choose one option from the select list when answering. Changing this Property to TRUE allows survey users to select and therefore to save more than one answer as response to a select list question.
Please note: it is advised that if you are using a SINGLE line select list, that multiple selections not be activated. If a survey user is given access to multiple selections, please create a multiple line select list (as described below)
- NUMBER OF OPTIONS SHOWN - this option allows for the creation of single or multiple line select lists, as shown in the examples below:

Single Line Select lists are created by default. If you wish to instead create a Multiple line select list, alter the numerical value shown in this Property field. In the example shown above, the number 4 has been entered allowing four lines worth of answers to be displayed. Additional answers may be accessed via a scrollbar on the right side of the select list.
- Text - This question type will create a single line, text field that will request free text from a survey user.
Answers given by a survey user in this type of question may be text or numbers (alphanumeric). If you wish to create a question like this, but accepting only numeric values (no spaces) from the survey user, please refer to Numeric questions above.
To create a Text question, choose Text from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by only Text Labels (which do not have a question number assigned) the Text question will still appear as number 1 in the survey.
- Each Text object requires a question. Enter the text for this in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Below the question, you will see an example of the text field, as it would appear to the survey user. There is no default text provided in this field - it is left blank. You may define default text to appear in the text field by typing it here.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, answering a Text question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- MINIMUM AMOUNT OF CHARACTERS - If the Text question being created is Mandatory, this field should be set to a number one (1) or higher - meaning that a survey user must enter at least one character in the text field in order to submit or proceed to the next page in the survey. Leaving this value as zero (0) allows the user to leave the field blank.
- MAXIMUM AMOUNT OF CHARACTERS - if you would like to restrict the length of survey user's answers to a Text question, this may be done by defining a Maximum Number of Characters the user may enter. To do define this number of characters, enter a number in this field. Once a survey user has entered the maximum, they will not be able to enter additional text into the answer field.
Please Note: if you are requesting an email address, you may wish to allow survey users greater flexibility in the length of their answer.
- SHOULD THE VALUE ENTERED BE UNIQUE? Use this option to define whether one survey user's answer must be different to all other survey user's answers to the same question. Setting this to NO allows more than one users to give the same response. This may need be set to YES (for example) if the Text question is requesting an email address, to prevent one email address's being added by to survey users.
- VALIDATION TYPE - Upon submitting or attempting to proceed to the next page in a survey, the details entered by a survey user into the Text question will be checked to make sure the information is in the format required. If not valid according to the validation selected, the survey user will be asked to attempt filling in the field again.

As the admin user, you may define what format this is. There are four types of validation that can be used for this type of field.
- Email - validation checks for a valid email address containing a name, the @ symbol, and a domain extension.
- Phone Number - validation checks for numbers and spaces only - no text.
- Characters - any combination of text and numbers is allowed for this validation type. This is selected as the default type of validation.
- Confirmation - when selected, instead of providing one single text field, the survey user will be presented with two. When attempting to submit or proceed to the next page in a survey, both fields must be filled in the same.

- Text Area - This question type will create a multiple line, text area that will request free text from a survey user. Answers given by a survey user in this type of question may be given in any alphanumeric format. To create a Text Area question, choose Text Area from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by only Text Labels (which do not have a question number assigned) the Text Area question will still appear as number 1 in the survey.
- Each Text Area object requires a question. Enter the text for this in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Below the question, you will see an example of the text area field, as it would appear to the survey user. There is no default text provided in this field - it is left blank. You may define default text to appear in the text area field by typing it here.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, answering a Text Area question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- CHARACTERS WIDE and LINES DOWN - Entering a number into these two fields allows you (the admin user) to define the size of the text area survey users are presented with. A default size of 50 characters wide and 10 lines down is given for all newly created Text Area questions. Increase or decrease these values as required.
- MAXIMUM CHARACTERS and MAXIMUM WORDS ALLOWED - Entering a number into these fields allows you (the admin user) to define how long a survey user's answer to the Text Area question may be. A default maximum length of 250 characters and/or 100 words is assigned to all newly created Text Area questions. Once a survey user has entered the maximum, they will not be able to enter additional text into the answer field.

- File Upload - This question type will create a File Upload field, through which a survey user may upload a file (in the form of a word document, an image, or a PDF) from their local computer. To create a File Upload question, choose File Upload from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by only Text Labels (which do not have a question number assigned) the File Upload question will still appear as number 1 in the survey.
- Each File Upload object requires a question, or instructions to the survey user. Enter the text for this in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, answering a File Upload question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
- ALLOW WORD DOC. IMAGE and PDF UPLOAD Fields - By default all three of these fields are set to Don't Allow ... Uploads. If left at the default values, survey users will not be able to upload. You, the admin user, must define what type of file survey users are allowed to upload when creating a File Upload question. Select from the select lists to allow one or more of the File Upload types for the question being created.
- IMAGE WIDTH and HEIGHT - If survey users have been given access to upload an image file, you (the admin user) may wish to define maximum the size, in pixels, that the uploaded images may be. Enter a numerical value in the fields provided.
- MAX FILE SIZE - You (the admin user) may wish to define maximum the size, in bytes, that the uploaded files may be. Enter a numerical value in the field provided.

- Date - This question type will create a multiple line, text area that will request free text from a survey user. Answers given by a survey user in this type of question may be given in any alphanumeric format. To create a Text Area question, choose Text Area from the select list when creating a question. You will be presented with a screen similar to the one shown below:

- First, choose which page in the survey this question will appear on and the order on this page it should be. Being a question - requesting input from the survey user - when placed onto a page in a survey, objects of this type are given a number in relation to all other questions currently on that page in the survey. If this is therefore the first question on a page then this question will be displayed as question number 1. Please note however that if such a question is preceded by only Text Labels (which do not have a question number assigned) the Date question will still appear as number 1 in the survey.
- Each Date object requires a question, or instructions to the survey user. Enter the text for this in the Question Label field. This field is also an Article Editor box, allowing you to format the question as required.
- Properties - Each question type has its own set of properties. All that request a response from the survey user may be defined as Mandatory (ie required).
- By default, answering a Date question is not mandatory, meaning that the survey user does not have to fill in an answer to the question in order to submit or proceed to the next page in the survey. If an answer is required by every user, the select list here should be set to Field is Mandatory.
Please Note: If the Date question is to be made mandatory, and users may be allowed to select the current date, you must set the Selected Date values to current, as shown in the following example:
